Animation highlighting best practice for older people with frailty

As part of our ongoing work for Newcastle-Gateshead Clinical Commissioning Group (CCG), and in particular their Care Home Vanguard team, we’ve produced an animation called Jackie’s Story – A System Response to Frailty. The animation will be shared widely within the CCG (and hopefully beyond) and used to supplement the vanguard’s work at conferences and presentations and add a human element, albeit through Jackie – a composite character created by the client from years of experience in the sector.

The Better Care Fund is a programme spanning both the NHS and local government which seeks to bridge the gaps between health and care services so that people can manage their own health and wellbeing, and live independently in their communities for as long as possible.
